How to fill the COTTON USA online verification form:

  1. Fill this questionnaire ONLINE (do not print and fill it manually!)
  2. Press button "Bestätigen/Confirm" at the end
  3. Save the Verification Form (PDF) on your PC/laptop
  4. If any information is missing – the system will not process! Just add missing data
  5. Print the verification form, check if all is correct
  6. Sign the form, put your stamp on it
  7. Scan it and send it to your customer or CCI Germany

 

Information zum Ausfüllen des Formulars:

  1. Abfrage komplett ONLINE ausfüllen (nicht ausdrucken und manuell ausfüllen)
  2. Auf  ”Bestätigen/Confirm” klicken
  3. Speichern Sie die Verification Form (PDF) ab
  4. Falls es nicht geht, fehlt noch eine Angabe – bitte ergänzen
  5. Drucken Sie die Verification Form aus und überprüfen nochmals die Angaben
  6. Unterzeichnen und mit Stempel versehen
  7. Senden Sie das gescannte Formular an CCI oder Ihren Kunden
