Fill this questionnaire ONLINE (do not print and fill it manually!)
Press button "Bestätigen/Confirm" at the end
Save the Verification Form (PDF) on your PC/laptop
If any information is missing – the system will not process! Just add missing data
Print the verification form, check if all is correct
Sign the form, put your stamp on it
Scan it and send it to your customer or CCI Germany
